trained on supported wearables, apps and Heads Up functionality.

- Full Health Coaching Service – $250 per patient per month
- 60 minutes of private coaching (split as needed)

- 1 hour 40 minutes health coach time per patient
- Laser Focused Health Coaching Service – $100 per patient per month
- 20 minutes of private coaching
- HIPAA compliant messaging for improved patient engagement
- 40 minutes of health coach time per patient
- Group Coaching Service $250/month up to 5 clients
- 1 group coaching session, 30-45 minutes
- HIPAA compliant messaging follow ups
- 1 hour 40 minutes health coach time per group
RPM add on option – Heads Up health coach works with practice’s insurance biller $150/hour.
Additional health coach services billed at $150/hr. Volume packages available upon request.
